Most RPG settings feature the same level-tiered setup. The PC’s home base is surrounded by an area populated by first level monsters, while the next concentric area is populated by second level monsters and so on. It’s usually a bit more complex than that, but that’s the basic model. Problem is, the basic model, and any models flowing from it are bullshit.
